ABSTRACT

Abstract Telehealth has been applied in the Unified Health System (SUS) as a tool for qualifying professionals and improving the healthcare provided to the population served by the system. The aim of the present study was to evaluate the impact of teleconsultations on the qualifications of the clinical approach of the oral health teams involved in primary health care (PHC) in the state of Rio Grande do Sul under the Telehealth Brazil Networks Programme. The sample population of the study was composed of 285 dentists and 132 oral health assistants belonging to the Family Health Teams. A chi-square test was used to investigate possible associations between the absolute number of teleconsultations performed and several independent variables, namely, gender, age, dental specialty, time since graduation, time working in the Family Health Strategy (FHS), employment status, type of training institution, familiarity with information technology, and work satisfaction at the health facility. The level of significance was p < 0.05. Use of the oral health teleconsultation service led to a reduction of more than 45% in the number of referrals to other levels of care. However, no significant association was found between the number of teleconsultations and the independent variables analysed. The use of teleconsultations proved to be a powerful tool for professional training, for increasing the effectiveness of PHC, and for improving the oral healthcare provided.

ABSTRACT

Resumo A garantia de atenção odontológica passa pelo desenvolvimento de práticas pautadas na vigilância em saúde, a fim de concretizar a integralidade. Objetivou-se avaliar a associação entre aspectos contextuais dos municípios brasileiros, características do processo de trabalho e a realização de um rol de procedimentos odontológicos curativos pelas equipes de saúde bucal (ESB). Trata-se de estudo exploratório transversal cuja coleta multicêntrica de dados se deu em 11.374 ESB avaliadas pelo Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica. Foi empregada regressão de Poisson multinível para obtenção da prevalência de realização de procedimentos odontológicos curativos, que foi de 69,51%. As variáveis contextuais e da equipe de saúde que se mantiveram associadas ao desfecho incluíram municípios cuja proporção de internações sensíveis à atenção básica foi menor que 28% e cuja proporção de exodontias foi menor que 8%; bem como ESB de modalidade II que tinham à disposição materiais, insumos e melhores processos de trabalho. Esta análise multinível, que considera o desempenho da atenção odontológica curativa no Brasil, aponta para um cenário de atenção odontológica preocupante.


Abstract Ensuring access to dental care services requires the development of healthsurveillance practices to ensure comprehensive health care. The objective of this study was toinvestigate the association between social and economic indicators of Brazilian municipalities, work process characteristics, and performance of a list of curative dental procedures by oral health teams. It involved an exploratory, cross-sectional study withmulticenter data collection from 11,374 oral health teams assessed by the National Program for Improvement of Access to and Quality of Primary Healthcare. Multilevel Poisson regression was used to obtain the prevalence of curative dental procedures, which was 69.51%. The social/economic and work variables that remained associated with the outcome included municipalities in which the proportion of primary care-sensitive admissions was below 28% and that of tooth extractions below 8%; and oral health teams classified as type II (including oral health assistant and technician) that had different materials available and better work processes. This multilevel analysis, which took into consideration the performance of curative dental care in Brazil, reveals a worrying oral healthcare scenario.

ABSTRACT

Objetivo: identificar a satisfação profissional de cirurgiões-dentistas atuantes na atenção básica do Sistema Único de Saúde em um município do sul do Brasil, que foi pioneiro na implantação de diretrizes municipais de saúde bucal. Sujeitos e método: para a coleta dos dados, foram utilizados componentes descritivos a partir de dados secundários obtidos por meio de um questionário aplicado para todos os cirurgiões-dentistas atuantes no referido serviço. Resultados: no total, 46 profissionais foram incluídos na amostra, sendo que 82,5% estavam satisfeitos em seu trabalho e, desses, o reconhecimento dos usuários do serviço foi o fator que os cirurgiões-dentistas mais associaram à felicidade. Já os fatores mais relacionados à infelicidade no trabalho são as críticas ao salário e os usuários não colaboradores; 97,8% acreditavam que seu trabalho fazia diferença na comunidade em que estavam inseridos. Conclusão: este estudo sugere que estabelecer diretrizes no processo de trabalho pode ter um impacto positivo na prática e na satisfação dos profissionais, e essas estratégias podem ser utilizadas como amparo para planejamento em gestão de saúde bucal de forma sistematizada. Objective: to identify the professional satisfaction of dentists working in the primary health care of the public health system, in a city in southern Brazil, which was a pioneer for implementing municipal oral health guidelines. Subjects and method: the data were collected by descriptive components from secondary data obtained through a questionnaire applied to all dentists working in the referred service. Results: a total of 46 professionals were included in the sample, whereas 82.5% were satisfied with their work, and the acknowledgment from service users was the factor that these professionals mostly associated with happiness. On the other hand, the factors most associated with unhappiness in the workplace are complaints of low wages and uncooperative patients. In addition, 97.8% of the professionals believed that their work made a difference in the community in which they were inserted. Conclusion: this study suggests that establishing guidelines in the work process may have a positive impact on the practice and the professional satisfaction and that these strategies may be used as a planning support for the systemic management of oral health. ABSTRACT

Resumo Objetivou-se identificar fatores associados à insatisfação com os serviços odontológicos utilizados entre adultos. Foram analisados os dados de 830 adultos participantes de um levantamento epidemiológico em saúde bucal. A variável dependente foi insatisfação com o serviço odontológico e as independentes foram reunidas conforme modelo teórico de Andersen e Davidson. As estimativas foram corrigidas pelo efeito de desenho amostral e foi conduzida Regressão Logística Binária. Cerca de 11% dos adultos encontravam-se insatisfeitos com o serviço odontológico utilizado. No modelo final, a insatisfação com os serviços odontológicos foi menor entre os adultos mais velhos (OR = 0,559) e entre os fumantes (OR = 0,332). Por outro lado, foi maior entre adultos que autopercebiam sua mastigação negativamente (OR = 2,804), que autopercebiam algum incômodo na região da boca, cabeça e pescoço (OR = 2,065) e que não tiveram acesso à informação sobre como evitar problemas bucais (OR = 3,020). Assim, os serviços precisam acessar as percepções e as expectativas manifestadas pelos usuários, além de prover informações em quantidade e qualidade adequadas, no contexto da “alfabetização em saúde”, a fim de se alcançar maior satisfação entre seus usuários.


Abstract This study aimed to identify factors associated with dissatisfaction with dental services among adults. It analyzed 830 adult participants of an epidemiological survey of oral health. The dependent variable was dissatisfaction with the dental service, and the independent ones were selected according to the theoretical model set forth by Andersen and Davidson (1997). Estimates were corrected by the sample design effect, and Binary Logistic Regression was carried out. It was found that about 11% of adults were dissatisfied with the dental service. In the final model, dissatisfaction with dental services was lower among older adults (OR = 0.559) and among smokers (OR = 0.332). On the other hand, it was higher among adults who self-perceived their chewing as negative (OR = 2,804), who self-perceived some discomfort in the mouth and head and neck region (OR = 2.065), and among those who did not have access to information on how to avoid oral problems (OR = 3.020). Therefore, the services need to access the perceptions and expectations expressed by users, and provide information in appropriate quantity and quality, in the context of “health literacy” in order to achieve greater satisfaction among its users.

RESUMO Objetivo O objetivo do estudo foi verificar o cumprimento das metas ambulatoriais dos Centros de Especialidades Odontológicas (CEOs) localizados na Região Sul do Brasil e fatores associados aos processos de trabalho. Método O estudo foi do tipo exploratório transversal. Para a coleta de dados foram consultadas as bases de dados do DATA/SUS, CNES, SIOPS, IBGE e foi enviado um questionário semiestruturado aos gestores dos CEOs. Para verificar a associação de fatores com o cumprimento das metas dos CEOs foi utilizado o Teste de Kruskal Wallis. Resultados Foram analisados 936 meses de produção ambulatorial distribuídos em 78 CEOs, a taxa de respostas dos questionários foi de 68 % e verificou-se baixo cumprimento de metas e que as variáveis: estado do Paraná, os municípios com porte populacional de 0-50 mil/hab e o quarto quintil dos gastos totais em saúde e PIB per capita foram associados ao cumprimento de metas dos procedimentos de periodontia e o tipo de CEO apresentou associação com metas cumpridas de procedimentos básicos. Discussão As análises das variáveis que podem estar associadas à produção ambulatorial dos CEOs são fundamentais para verificar questões de acesso da população e aplicabilidade dos recursos públicos na área da saúde. Conclusões O estudo sinaliza para o baixo cumprimento de metas dos CEOs, entretanto aponta que podem haver fatores importantes associados a maior número de meses de metas cumpridas.(AU)


ABSTRACT Objective To evaluate the association of work factors with goals set in specialized dental centers (SDCs) of Southern Brazil. Methodology Cross-sectional exploratory study, in which the databases DATASUS, IBGE, SIOPS were used and complemented with an online questionnaire (FORMSUS). The questionnaire was answered by SDC managers. The Kruskal Wallis test was utilized to determine association of factors. Results Nine hundred and thirty six months, corresponding to 78 SDCs, were analyzed; the response rate of the questionnaires was 68%. It was found that the variables Paraná State, municipalities with population size of 0-50 thousand/inhabitants, and the fourth quartile of total health expenditure and Gross National Product per capita were factors associated with goals achievement of periodontal procedures. The type of SDCs showed association with goals achievement of basics procedures. Discussion Analyzes of the variables that may be associated with outpatient treatment in of SDCs are fundamental to verify issues regarding population access and applicability of public resources in the health area. Conclusion The study indicates low achievement goals by outpatient care delivery by specialized dental centers, which may indicate important factors associated with higher number months for achieving goals.(AU)


RESUMEN Objetivo El objetivo del estudio fue verificar: el cumplimento de las metas de tratamientos en las clínicas dentales especializadas (CDEs) en la Región Sur de Brasil y, los factores asociados a los procesos de trabajo. Método El estudio fue exploratorio transversal. Para la recolección de datos fueron consultadas las bases del datos provenientes de DATA/SUS, CNES, SIOSP, IBGE y fue enviado un cuestionario semiestructurado para los gestores de las CDEs. Para comprobar la asociación de los factores con el cumplimento de las metas de las CDEs se realizó el test de Kruskal Wallis. Resultados Fueron analizados 936 meses de tratamiento ambulatorio distribuidos en las 78 CDEs y la tasa de respuestas de los cuestionarios fue de 68%. Las variables: estado de Paraná, municipios con número de ciudadanos de 0-50 mil/hab, el cuarto quintil de los gastos totales en salud y PIB per cápita fueron asociados con el cumplimento de las metas de los procedimientos de periodoncia y, el tipo II de CDE presentó asociación con el cumplimento de las metas de los procedimientos básicos. Discusión El análisis de las variables que pueden estar asociadas al tratamiento ambulatorio en las CDEs es fundamental para comprobar tanto los problemas de acceso de los habitantes como la aplicabilidad de los recursos públicos en el área de la salud. Conclusiones El estudio indica un bajo cumplimento de las metas de tratamiento de las CDEs, entretanto apunta que pueden haber factores importantes asociados al mayor número de meses en las metas cumplidas.(AU)

ABSTRACT

Objective: To verify user satisfaction with the public oral health services in its different dimensions of care and relate them to the socioeconomic and demographic conditions. Material and Methods: Cross-sectional survey, developed with users interviewed of the public oral health services under the Primary Health Care of a Brazilian city (n=461). The associations of satisfaction with the socioeconomic and demographic conditions were analyzed using the chi-square test. Questions concerning satisfaction with the service were grouped into five domains - 'physical structure', 'relation and communication', 'information and support', 'health care' and 'organization of services' - analyzed using the Kruskal-Wallis Test. Results: Elderly individuals with low education and male, tended to be more satisfied with the services. The users reported being satisfied with the service, that were unhappy, referred to the time and the form to schedule a consultation, the time expected to be attended and the resolving capacity of oral health problems, as the causes. The overall satisfaction index was 0.8. The top rated domains were 'physical structure' (0.9) and 'relation and communication' (0.85). The 'service organization' domain received the worst evaluation (0.71) and when compared to the others domain presenting a significant difference. The 'physical structure' domain was significantly highlighted from the domains of 'oral health care' (0.78) and 'information and support' (0.78). The 'relation and communication' domain, also significantly excelled over the areas of 'oral health care'. Conclusion: The users rated the aspects of health care in a very positive way. The areas of highest user satisfaction were 'physical structure' and 'relation and communication'. However, the need for directing production processes in health became evident, especially regarding the organization of services, the domain with the worst rating.

ABSTRACT

A presença polarizada de cárie dentária e de doença periodontal, ainda, caracterizam a saúde bucal do Brasil, nos dias de hoje. São altos os índices de extrações dentárias e procedimentos restauradores, especialmente em crianças. Diante deste quadro epidemiológico preocupante, emergem as ações de promoção em saúde bucal no âmbito escolar. A escola apresenta uma infraestrutura que facilita a aquisição de novos conhecimentos, bem como a construção de hábitos saudáveis. No entanto, são raros os trabalhos que avaliam a efetividade dos programas de promoção em saúde bucal neste ambiente. Dessa maneira, este estudo objetivou avaliar de forma comparativa a eficácia do Programa de Promoção em saúde bucal, em Escolas Municipais de Ensino Básico, desenvolvido há 17 anos pela Faculdade de Odontologia de Araçatuba, direcionado aos pré-escolares e seus pais ou cuidadores, e equipe pedagógica. Dois grupos foram formados (grupo intervenção e grupo controle). O grupo intervenção foi constituído por 5 escolas que participaram das atividades do programa de promoção em saúde e o grupo controle foi constituído por 5 escolas que nunca haviam participado das atividades do programa. Os dados foram coletados por meio de questionários e entrevistas com os três atores sociais envolvidos no processo (pré-escolares, pais ou cuidadores e equipe pedagógica). Os dados foram analisados utilizando os programas Epi Info 3.5.1 e SPSS 22.0. Os resultados do estudo mostraram que os pré-escolares do grupo intervenção possuem mais conhecimento sobre saúde bucal que as crianças do grupo controle (p<0,001). Em contrapartida, não foram encontradas diferenças significantes entre os grupos intervenção e controle dos pais ou cuidadores, e equipe pedagógica. Desta forma, pode-se concluir que o maior conhecimento em saúde bucal estava significativamente associado ao programa de promoção em saúde bucal da Faculdade de Odontologia de Araçatuba, para os pré-escolares. The Brazilian oral health is still characterized by the polarized presence of dental caries and periodontal disease. The tooth extraction and restorative procedures rates are high, especially in children. Faced with this alarming epidemiological situation, oral health promotion activities are emerging in the school environment. The school offers an infrastructure that facilitates the acquisition of new knowledge and the development of healthy habits. The studies evaluating the effectiveness of oral health promotion programs in schools are scarce. Consequently, this study aimed to assess comparatively the efficacy of Oral Health Promotion Program in nursery schools, developed 17 years ago by Araçatuba Dental School and targeted to preschoolers and their parents / carers and teaching staff. Two groups were formed (intervention group and control group). The first one was consisted of five schools that received the oral health promotion program activities and the control group was consisted of five schools that had never received the program activities. Datas were collected by questionnaires and interviews with the three social agents involved in the process (preschoolers, parents / carers and teaching staff). The datas were analyzed using the statistical software Epi Info® 3.5.1 and SPSS® 22.0. The results revealed that preschoolers in intervention group have more knowledge in oral health than children in control group (p < 0.001). However, no significant differences were found between the intervention and control groups of parents / carers and teaching staff. Thus, the greatest knowledge on oral health was significantly associated to oral health promotion program of Araçatuba Dental School, among preschoolers. ABSTRACT

This study aimed at identifying the prevalence of the negative evaluation of dental services among elderly Brazilians and at investigating whether the prevalence was higher among those using public or philanthropic provider services than among those paying privately or using private health plans. Additionally, factors associated with this negative assessment were identified. Interview and survey data were collected in the residences of participants by trained and calibrated examiners as part of a national epidemiological survey of oral health conditions of the Brazilian population in 2002/2003. The dependent variable was obtained in response to questions regarding whether the participant had ever used dental services, the frequency of use, and the quality of this service. Potential responses to the questions regarding the quality of service were very poor or poor, fair, and good or very good. The main independent variable was the system of health care used with potential responses being health plan or private, public, and philanthropic services. We conducted univariate (linear tendency χ2test) and multiple descriptive analyses, and the partial proportional Odds model for ordinal logistic regression. Among the elderly, 196 (3.7%) evaluated the provided services negatively (very poor or poor). Participants with the following responses were more likely to evaluate the services negatively: those who had used public or philanthropic services, men, those with higher education, the ones who had not received information about preventing dental problems, those who perceived pain in their teeth and gums in the last six months, and those who self-reported their oral health and speech was poor. In conclusion, elderly Brazilian users of public and philanthropic services were more likely than users of private or insurance-based plans to evaluate their dental services negatively, regardless of the other investigated variables.


Objetivou-se avaliar a prevalência de avaliação negativa dos serviços odontológicos e constatar se ela foi maior entre usuários de serviços públicos ou filantrópicos do que entre idosos brasileiros usuários de serviços particulares ou de planos de saúde. Além disso, foram identificados os fatores associados a tal avaliação negativa. Foram utilizados dados do levantamento epidemiológico das condições de saúde bucal da população brasileira de 2002/2003, obtidos por meio de entrevistas e exames conduzidos nos domicílios por examinadores treinados e calibrados. A variável dependente foi obtida a partir das respostas sequenciais às perguntas: "Já foi ao dentista alguma vez na vida?; Há quanto tempo? e Como avalia o atendimento?", categorizadas em péssimo/ruim; regular e bom/ótimo. A principal variável independente foi o "Sistema de atenção à saúde utilizado (plano de saúde/particular; público/filantrópico)". Foram conduzidas análises descritivas, univariadas (prova de tendência linear do χ2) e múltiplas, utilizou-se a regressão logística ordinal de Odds proporcionais parciais. Dentre os idosos, 196 (3,7%) avaliaram negativamente (péssimo/ruim) os serviços odontológicos. A avaliação negativa foi superior entre aqueles que usaram serviços públicos/filantrópicos; homens; com maior escolaridade; que não receberam informações sobre como evitar problemas bucais; que autoperceberam dor nos dentes e gengivas nos últimos seis meses, a saúde bucal e a fala como ruim/péssima. Conclui-se que a avaliação negativa foi maior entre usuários de serviços públicos e filantrópicos independentemente das demais variáveis investigadas.

ABSTRACT

Com o objetivo de testar abordagem metodológica para avaliação da cobertura dos serviços de atenção básica em Salvador, Bahia, Brasil (2000 a 2007), foi realizado um estudo de agregados espacial e temporal, tendo como unidades de análise o município e seus 12 Distritos Sanitários. Foram estimadas as coberturas real e potencial de diversos procedimentos realizados na atenção básica. Para a estimativa das coberturas, além do padrão ministerial, foi elaborado um segundo padrão com o objetivo de estabelecer uma aproximação com as necessidades de saúde da população. Os dados foram obtidos do Sistema de Informações Ambulatoriais do Sistema Único de Saúde (SIA-SUS). Foram encontradas baixas coberturas assistenciais no período analisado, a despeito da ampliação discreta verificada tanto na cobertura real de consultas médicas (7,8%), quanto na produção de ações básicas de enfermagem (66,7%). Já as consultas odontológicas apresentaram redução em 40%. Foram discutidas tanto as possibilidades de uso da metodologia para o monitoramento da cobertura, como as suas limitações tendo em vista as deficiências dos sistemas de informação.


In order to test a method for evaluating coverage of primary healthcare services in Salvador, Bahia State, Brazil (2000-2007), a study of spatial and temporal clusters was performed, with the city and its 12 health districts as the analytical units. The real and potential coverage rates were estimated for various primary care procedures. Estimation of coverage rates used both the Ministry of Health guideline and a second standard aimed at establishing an approximation to the population's health needs. The data were obtained from the Outpatient Information System of the Unified National Health System (SIA-SUS). Low coverage rates were observed, despite a slight increase in real coverage for medical consultations (7.8%) and basic nursing care (66.7%). Meanwhile, dental consultations showed a 40% reduction. The study discussed the possibilities for using the methodology to monitor coverage and its limitations given the deficiencies in the information systems.

Introdução - O Programa Preventivo para Gestantes e Bebês (PPGB) é uma atividade de extensão do curso de Odontologia da Universidade Federal do Piauí (UFPI), cujo objetivo é o atendimento odontológico de gestantes e crianças na faixa etária de zero a36 meses. O presente estudo objetivou avaliar o perfil e condições de saúde bucal das gestantes atendidas no PPGB no período de março de 2003 a março de 2009. Material e Método - Foram selecionados 933 prontuários que continham as informações necessáriasao estudo, como valores dos Índices de Placa Corada, Sangramento Gengival e CPO-D, além de aspectos sociodemográficos. Os dados foram transcritos para ficha elaborada parao estudo e digitadas em planilha do programa SPSS versão 17.0. Para análise estatística utilizou-se os testes Kruskal-Wallis, Dunn e qui-quadrado com nível de significância de 95%. Resultados - A amostra era composta em sua maioria por gestantes jovens (médiade idade 25,4 anos), escolaridade média de 9,6 anos de estudo formal, renda familiar mensal entre um e dois salários mínimos. A maioria estava na primeira gestação e buscou atendimento odontológico no segundo trimestre da gravidez por procedimentos preventivo-restauradores. As gestantes não relataram presença de sangramento gengival antes nem durante a gravidez. O Índice de Placa Corada médio foi de 70% e o CPO-D foi de 8,03, 10,55 e 15,43 para as faixas etárias até 19 anos, 20 a 29 anos e maior de 30 anos,respectivamente. Conclusão - Pode-se concluir que a maioria das gestantes necessitava de tratamento restaurador de baixa complexidade.


Introduction - The Preventive Program for Pregnant Women and Babies (PPPWB) is an outreach activity of the dental clinic of Federal University of Piauí (UFPI), whose objective is the dental care of pregnant women and children aged zero to 36 months. This study aimed to evaluate the profile and the oral health status of pregnant women in PPPWB from March 2003 to March 2009. Material and Methods - 933 records were selected containingthe information needed to study, as values of the index card flushed, gingival bleeding and DMFT, as well as socio-demographic factors. Data were transferred to the schedule established for the study and entered into the spreadsheet using SPSS 17.0. For statistical analysis were used the Kruskal-Wallis, Dunn and chi-square test with significance level of 95%. Results - The sample consisted mostly of young pregnant women (mean age 25.4years), average schooling of 9.6 years of formal schooling, monthly family income between one and two minimum wages. Most were in the first pregnancy and sought dental treatment in the second trimester of pregnancy for preventive, restorative procedures. The womendid not report the presence of gingival bleeding before or during pregnancy. The Plaque Index Flushed averaged 70% and DMFT was 8.03, 10.55 and 15.43 for age 19 years, 20 to 29 years and older than 30 years, respectively. Conclusion - It can be concluded that most pregnant women in need of restorative treatment of low complexity.

ABSTRACT

Este trabalho discorre a respeito da avaliação da atenção secundária no âmbito da vigilância em saúde. Estudo descritivo, de caráter avaliativo normativo. Para análise de desempenho, foram utilizados dados secundários, com base na série histórica da produção de procedimentos odontológicos realizados nos CEO implantados no país e registrados pelo SIA/SUS no ano de 2007, e dados primários, pela visita in loco de 10% dos serviços implantados, com uso de questionários dirigidos à equipe profissional do CEO. Dentre os CEO analisados, observou-se que na maioria das regiões houve dificuldade no cumprimento das metas relativas à quantidade de procedimentos a serem informados no SIA/SUS. Sendo a Região Norte com menor percentual de serviços implantados. O indicador Cumprimento da Atenção Secundária em Saúde Bucal foi de 64,4%. Os CEO tipo III tiveram melhores resultados para o desempenho e cumprimento das metas. Foi evidenciada a necessidade de revisão no marco legal para a implantação dos CEO pela readequação dos critérios e normas, como também definições de novos padrões e cumprimento de metas para avaliação e monitoramento desses serviços.


This article discusses the evaluation of secondary care in the area of health surveillance. This was a descriptive and normative/evaluative study. Performance analysis drew on secondary data, based on a historical series of dental procedures conducted at the specialized dental clinics implemented in Brazil and recorded by the Outpatient Information System of the Unified National Health System (SIA/SUS) in 2007, as well as primary data from site visits to the clinics, based on questionnaires completed by clinic staff. Performance of the clinics was poor in most regions of the country, and the North of Brazil had the lowest percentage of specialty services implemented. The indicator "Performance of Secondary Care in Oral Health" was 64.4%. The type 3 specialty clinics showed better results in terms of performance and achievement of targets. The study showed the need to review the legal framework for implementing specialized dental clinics by adjusting the criteria and norms, as well as definition of new standards for achievement of goals in the evaluation and monitoring of these services.

ABSTRACT

The objective of this article is to describe and analyze the development of indicators used to identify strengths and deficiencies in public dental healthcare services in the municipality of Cambé, Paraná. The methodology employed was a historical-organizational case study. A theoretical model of the service was developed for evaluation planning. To achieve this, information was collected from triangulation of methods (interviews, document analysis and observation). A matrix was then developed which presents analysis dimensions, criteria, indicators, punctuation, parameters and sources of information. Three workshops were staged during the process with local service professionals in order to verify whether both the logical model and the matrix represented the service adequately. The period for collecting data was from November 2006 through July, 2007. As a result, a flowchart of the organization of the public dental health service and a matrix with two-dimensional analysis, twelve criteria and twenty-four indicators, was developed. The development of indicators favoring the participation of people involved with the practice has enabled more comprehensive and realistic evaluation planning.


O objetivo do presente artigo é descrever e analisar o desenvolvimento dos indicadores utilizados para se identificar fortalezas e deficiências de serviço público de odontologia no município de Cambé, Paraná. A metodologia usada foi o estudo de caso histórico-organizacional. Para o planejamento da avaliação foi desenvolvido o modelo-lógico do serviço. Para tanto, foram coletadas informações a partir da triangulação de métodos (entrevistas, análise documental e observação). Na sequência, foi desenvolvida uma matriz que apresenta dimensões de análise, critérios, indicadores, pontuações, parâmetros e fontes de informações. Três oficinas de trabalho foram realizadas com profissionais do serviço local visando melhor adequação do modelo-lógico e da matriz à realidade do serviço. O período de coleta de dados foi novembro de 2006 a julho de 2007. Como resultado, obteve-se um fluxograma da organização do serviço de odontologia e uma matriz com duas dimensões de análise, doze critérios e vinte e quatro indicadores. O desenvolvimento dos indicadores, privilegiando a participação dos sujeitos envolvidos com a prática, proporcionou o planejamento de uma avaliação abrangente e realista.

ABSTRACT

Este estudo teve como objetivo analisar o processo de trabalho desenvolvido pelas equipes de saúde bucal do Distrito Sanitário VI do município de Recife (PE), no ano de 2006. Trata-se de um estudo descritivo de abordagem qualitativa, em que foram utilizados dados secundários obtidos de uma pesquisa anterior realizada na cidade do Recife, onde o Distrito Sanitário VI está localizado. Para isso, foram utilizadas quatro entrevistas semiestruturadas com o coordenador de saúde bucal distrital, dois cirurgiões-dentistas e uma enfermeira, que atuavam na Estratégia de Saúde da Família. Os resultados mostraram que algumas práticas realizadas pelas equipes ainda são incipientes, como o levantamento epidemiológico, a referência e contrarreferência dos pacientes e o monitoramento e avaliação das ações. Entretanto, o avanço na visão preventiva e na prática de educação em saúde dos profissionais foi evidenciado através das entrevistas. Conclui-se que, apesar das dificuldades para o cumprimento das práticas estabelecidas pela Política Nacional de Saúde Bucal, percebe-se uma mudança do modelo de atenção em saúde bucal, sendo necessários ajustes e correções nas atividades das equipes, além de uma maior participação dos demais níveis de gestão para o exercício pleno da saúde bucal na Estratégia de Saúde da Família.


The aim of this study was to analyze the work process developed by the oral health teams of the VI Sanitary District of Recife, Pernambuco State, in 2006. It is a descriptive study of qualitative approach in that secondary data were obtained of a previous research accomplished in Recife, where the Sanitary District VI is located. Four semi-structured interviews were accomplished with the oral health coordinator, two surgeon-dentists and a nurse that acted in the Family Health Strategy. The results showed that some practices accomplished by the teams are still incipient such as the epidemic survey, patient reference and counter reference, and the control and evaluation of the actions. However, the progress in the preventive vision and in education practice in the professional health became evidenced through the interviews. In conclusion, in spite of the difficulties for the execution of the practices established by the National Policies of Oral Health, it is noticed a change in the attention model in oral health, being necessary some adjustments and corrections in the activity of the teams, besides a larger participation of other administration levels for the full exercise of oral health in the Family Health Strategy.

ABSTRACT

Increasing the quality of the services provided in a Dental School can raise the satisfaction level of patients and consequently increase the level of their oral health. This study was conducted to evaluate the quality of dental care and services provided to patients referred to a Dental School in Kerman, Iran. In this qualitative study, face-to-face, in-depth interviews were conducted with 41 participants #&091;25 patients (P), 5 nurses (N), 6 dental academic staff (AS), and 5 dental students (S)#&093;. Then, the interviews were transcribed and analyzed, using content analysis of data. Data analysis in qualitative research involves breaking down the data and searching for codes and categories that are then reassembled to form themes. Both positive and negative themes emerged. Positive themes included: good infection control, service accessibility, patient appointments and visits were not assigned on merit, precise examinations, and comprehensive treatment plans. Negative themes included: long wait time, lack of options to pass waiting time, such as newspapers and television, an insufficient number of nurses, and not enough professors for supervision. In addition, the results of this study show that the patients and dental staff have high expectations in relation to dental services, and that implementation of these expectations would increase the overall satisfaction with and the quality of the level of services. Finally, some recommendations for improving services in the Kerman Dental School were given to the managing team of the Dental School.

ABSTRACT

Foi objetivo deste trabalho avaliar o serviço de saúde bucal, através do estudo das percepções dos usuários, do município de Grão Mogol - norte de Minas Gerais, Brasil. Optou-se pela metodologia qualitativa, utilizando entrevista semi-estruturada. Foram levantados aspectos sobre percepção do processo saúde/doença bucal; conhecimentos, práticas e valor atribuído à saúde bucal; acesso aos serviços; relação profissional/paciente e assistência recebida. Obtiveram-se as idéias centrais em cada tema abordado nas entrevistas, sendo posteriormente agrupadas e analisadas, constituindo a análise final do estudo. Observou-se que o serviço de saúde bucal, no município, vive período de mudanças, mas existe ainda uma prática iatrogênica-mutiladora. No entanto, a prevenção e a promoção da saúde ganham espaço entre as ações de saúde, atuando como importantes fatores para a melhoria da assistência à saúde bucal e consequente melhoria na qualidade de vida da população. Os usuários percebem a evolução do serviço, mas admitem a necessidade de se organizar e definir melhor o acesso aos serviços e otimizar a comunicação profissional/paciente. A população começa a valorizar a saúde bucal como algo importante dentro das condições de vida de cada indivíduo.


The objective of this study is to evaluate the oral health services in the city of Grão Mogol, Brazil by analyzing the user’s perceptions. It was chosen the qualitative methodology using a semi-structured interview. The aspects investigated included the perception of the process of oral health/illness; knowledge, practices and value attributed to oral health; access to services; professional/patient relationship and assistance received. The interviews were analyzed and the central ideas extracted, which were considered in a final analysis of the study. It was observed that the oral health service in this city is going through a period of changes, but it still exist an iatrogenic-mutilator practice. However, health prevention and promotion gains space within health actions, acting as important factors to enhance oral health assistance and resulting improvement of the population quality of life. Users can feel the evolution of the service, but admit the necessity of more organization and definition on the access to services as well as an improvement on the communication professional/patient. The population starts to value the oral health as something important within the life conditions of each individual.

Objetivando avaliar os Centros de Especialidades Odontológicas de Pernambuco, Brazil, realizou-se estudo exploratório, utilizando dados secundários da produção ambulatorial 2006, critérios e normas instituídos para implantação destes serviços. Descreveu-se o cumprimento global das metas dos Centros de Especialidades Odontológicas, variáveis de caracterização dos serviços e dos municípios. Foram utilizadas análises do tipo descritiva e do tipo correlacional não paramétrica (coeficiente de Spearman), considerando-se o nível de significância de 5 por cento. Dos 22 Centros de Especialidades Odontológicas, 40,9 por cento obtiveram um desempenho bom, destacando que 81,8 por cento cumpriram a meta para o subgrupo atenção básica em contraste com 13,6 por cento que cumpriram a meta do subgrupo cirurgia oral menor. Dentre as variáveis independentes analisadas, apenas o porte populacional e o índice de desenvolvimento humano do município foram correlacionados ao desempenho dos Centros de Especialidades Odontológicas, quanto menor a cidade (r = 0,678; p < 0,001) e menor desenvolvimento humano (r = 0,599; p < 0,001), piores desempenhos foram observados. O estudo sugeriu que a normatizaçao para implantação e funcionamento dos Centros de Especialidades Odontológicas deva ser monitorada e avaliada para garantir uma melhor qualidade dos serviços para população.


This exploratory study aimed to evaluate the Specialized Dental Care Centers in Pernambuco State, Brazil, based on secondary outpatient treatment data from 2006 and standards and criteria adopted to implement these services. The study describes the overall achievement of targets by these specialized dental clinics and variables for characterization of the dental services and municipalities (counties). The methodology included descriptive analysis and non-parametric correlation analysis (Spearman coefficient), with significance set at 5 percent. Of the 22 specialized dental clinics, 40.9 percent showed good performance, and 81.8 percent met the target for basic dental care, as compared to 13.6 percent that met the target for minor oral surgery. Among the independent variables, only population size and Municipal Human Development Index (MHDI) were correlated with the performance of the specialized dental clinics: the smaller the city (r = 0.678; p < 0.001) and the lower the MHDI (r = 0.599; p < 0.001), the worse the performance. The findings suggest that standardization of the implementation and functioning of specialized dental clinics should be monitored and evaluated in order to ensure better quality of services for the population.

ABSTRACT

This paper reviews the historical development of public health policies in Brazil and the insertion of oral health in this context. Since 1988, Brazil established a Unified National Health System ("Sistema Único de Saúde" - SUS), which was conceived to assure access to health actions and services, including oral health. However, a history of lack of access to health services and the health problems faced by the Brazilian population make the process of building and consolidating the SUS extremely challenging. Since 2004, the Oral Health National Policy has proposed a reorientation of the health care model, supported by an adaptation of the working system of Oral Health teams so that they include actions of health promotion, protection and recovery. Human resources should be prepared to act in this system. The qualifying process must take in consideration knowledge evolution, changes in the work process and changes in demographical and epidemiological aspects, according to a perspective of maintaining a balance between technique and social relevance.

En mayo del 2003 se inaguró la clínica odontológica Sonrisa Total, contando con una ubicación privilegiada, equipamiento con tecnología de punta, profesionales especializados y sobre todo con una visión empresarial, capáz de proveer servicios odontológicos de calidad.Con la primeras experiencias obtenidas, fue surgiendo la necesidad de incursionar en el modelo de aseguramiento, sin tener como base ninguna formación en el tema, se estableció el conjunto de prestaciones y el precio mensual por afiliado, impulsando la necesidad de generar e incrementar la demanda.Con la firna de los primeros convenios institucionales y contando ya con 200 afiliados se inició en ocubre con el proceso de atención, simultáneamente se realizó el diagnóstico y tratamiento de los pacientes beneficiarios, optimizando el tiempo y los recursos, se concluyó con el tratamiento de los afiliados..
